There are several instances where I need either a yes/no, fill in the blank type form along with an order form. One example is a tournament RSVP form where folks can indicate whether they are attending or not, volunteer to help with chaperoning, snacks, etc., and then pay for their players tournament fee.....THIS WOULD BE GREAT!!
